What we catch

Every form failure. Before the inspector finds it.

§ 4.10.3

Whiteout & correction tape

Any form of cover-up — whiteout, correction tape, scribble — is an automatic rejection. Single line-through with initial and date is the only acceptable correction.

Automatic rejection if found
§ 4.3.4.6

Pencil entries

All signatures and entries must be in permanent ink. Pencil marks are erasable and compromise document integrity — any pencil entry is grounds for rejection.

Caught on photo scan
§ 4.13.1

Missing heat numbers

Every traceable material must have a documented heat/trace number. Missing, illegible, or undocumented heat numbers make the entire batch non-conforming.

Entire batch flagged
§ 4.3.4.8

Self-inspection

An inspector cannot inspect their own work. Violation of independence requirements can invalidate an entire inspection record, regardless of quality of work performed.

Entire record invalidated
§ 4.10.1

Illegible entries

All entries must be clearly legible. Poor handwriting, smudged ink, or low-contrast marks are grounds for rejection — even if the information is technically correct.

Rejected at QC review
§ 4.3.4.6

Missing signatures & initials

Every line item requiring inspector authentication must have a distinct, legible signature or initials. Blank fields mean the inspection cannot be verified.

Form cannot be accepted
